The following performance contracting forms are available for use by the divisions, campuses, directorates, faculties, schools, departments and sections within the University.

• Departmental PC-WP Template F-2-53-2-1 (Revised) This template is used for preparing the harmonized departmental performance contract and work plan.
• Departmental-PC and Work Plan Reporting-Form F-2-53-2-2This form is to be used for submitting quarterly reports on the  harmonized departmental performance contract and work plan.
